BOARDING INFORMATION
Brook-Ellis Pet Hospital offers boarding for your pet! Whether it's for a day while your pet is recovering from an illness or if you are planning a vacation, we are here for your pets boarding needs. Your pets receive the utmost care while spending time with us at Brook-Ellis. They are treated like part of our family with multiple walks throughout the day, along with any extra treatment or care they might require while visiting with us. Contact us today for reservations for your pets boarding dates. In order to save time when dropping your pet off for boarding, print out the PDF file below and fill it out. Be sure to bring it with you on the day of your pets' arrival at Brook-Ellis Pet Hospital. For health and safety reasons, new clients and new pets of existing clients, are required to have a physical exam by the Doctor before your pet will be able to board with us.

DROP OFF EXAMS
Does your pet need to be seen by the Doctor, and your schedule is just too hectic? Brook-Ellis Pet Hospital offers "Drop off" exams. To make it easier we have provided the Drop off exam form below. Be sure to print it out and bring the completed form with you when you drop your pet off for their appointment. There is an additional day hospital fee for these exams and appointments are required for this service so be sure to call ahead!

SURGERY AUTHORIZATION
Is your pet scheduled to have surgery at Brook-Ellis Pet Hospital? When dropping your pet off at the prearranged time for their surgery, make sure to print and complete the Surgery Authorization form and leave it with our receptionist when you bring your pet in for their appointment.
